We are located at 132 Hospital Drive, Watertown, Wisconsin. This is just across the street from Watertown Memorial Hospital. From anywhere in the area, just follow the blue hospital signs and you'll easily find our clinic.

APPOINTMENTS


For your convenience, we offer scheduled appointments for physicals, well baby and child care, acute and chronic medical problems and other non-emergencies. We schedule appointments between 8:00 AM and 7:00 PM Monday through Wednesday. On Thursday and Friday, we schedule appointments from 8:00 AM to 5:00 PM. The receptionist is available to answer the phone on weekdays at 7:30 AM.

Patients with minor emergencies or more urgent needs will be seen without an appointment. However, if possible, it is best to call the office before coming if immediate attention is not necessary.

Our clinic uses the "open access" method of scheduling. This method allows us to keep large blocks of openings available each and every day. Our goal is to see every patient at a date and time that is convenient for them. By using this method, same day appointments are the rule, not the exception. For example, if a patient is due for a complete physical and finds that he or she has an unexpected day off, the physical could be performed that same day.

All of the providers will attempt to stay on schedule. We realize that your time is as valuable as ours. Unfortunately, patients often have complex medical problems that take more time than anticipated. In addition, emergencies which can cause us to be off schedule. We hope you will be understanding and apologize for any inconvenience.

EMERGENCIES


In addition to our extended hours, Dr. Turke or a colleague are available 24 hours a day 365 days a year. This physician can be reached after hours by calling (920)261-4210. Please identify yourself as a patient of Dr. Turke. Dr. Turke or an associate on call will return your call shortly.

MISSION STATEMENT


The goal of the medical providers and employees of Turke Family Medicine S.C. is to provide cost effective, comprehensive medical care in a prompt and courteous fashion and to improve the health of our patients through education and preventive care. It is also our goal to help our patients obtain appropriate consultation as indicated. We will strive to attain excellence in delivery of care and patient relations.

TELEPHONE CALLS


For urgent problems or questions feel free to call the office at any time. Dr. Turke, one of the physician assistants or one of the nurses are available to answer your questions and to provide refills of medications as indicated.

Generally, Dr. Turke or one of the physician assistants will return your call after reviewing their messages. Depending upon the nature of the problem, they may take your call immediately.

If you have an urgent question or an emergency after hours, please call the after hours number(920)261-4210. For non-urgent calls, medication refills, billing questions, lab results, or referral questions, please contact our office during regular business hours. MEDICAL PROVIDERS


Photo of Dr. TurkeTerry L. Turke, M.D., is a board certified family practitioner. He received his B.S from the University of Wisconsin where he graduated with Honors and was elected to Phi Beta Kappa. He also received his M.D. degree from the University of Wisconsin, also graduating with honors. After completing his residency in Family Practice at the U.W., he began practice in Watertown, Wisconsin.

He is a member of the American Academy of Family Practice, as well as the AMA, State Medical Society of Wisconsin and the Jefferson County Medical Society. He is a member of the medical staff of Watertown Memorial Hospital and has served in numerous capacities, including Chief of Staff. He has served as chairman of the Quality Council and has served on the Executive Committee of the hospital as well. He has served on the AAFP Technical Panel on Computerization and is a legislative liaison for the Wisconsin Academy of Family Practice. He has served as a Preceptor in the University of Wisconsin Physician Assistant Program and is an Assistant Clinical Professor of Family Medicine at the University of Wisconsin Medical School. He has served as a Mentor for the Clinical Continuum Program at the Medical College of Wisconsin. In addition, he participates in the summer externship for medical students sponsored by the Wisconsin Academy of Family Practice. He is currently serving as president of the Watertown Physician Hospital Organizaton

Dr. Turke's special interests in medicine include issues of quality care, the use of computers in medicine, especially the electronic medical record and the use of physician extenders to increase access to primary care.

Dr. Turke is married and has 4 children. His wife, Janice, is a registered nurse and works part time at the clinic as well. His hobbies include golf, walking, reading, bluegrass and computers. He is also trying to teach himself to play the 5-string banjo.

Jessica JaekelsJessica L. Jaekels, PA a physician assistant, recently joined Turke Family Medicine S.C. Jessica received her Bachelor of Science degree in Biomedical Sciences from Marquette University. She also received her Master of Physician Assistant Studies from Marquette University, graduating with honors. She was drawn to family practice because of her desire to care for patients of all ages and needs. Her special interests in medicine include cardiology and pediatrics. Her philosophy of care is to provide each patient with the best level of care possible, and to always have time to listen and educate. A native of Green Bay, she currently lives in Pewaukee. She is the oldest of four daughters. She enjoys all outdoor activities, especielly biking and soccer.

Teresa GrimesTeresa Graef Grimes, PA-C. is the most recent Physician Assistant to join Turke Family Medicine S.C. A native of Wisconsin, Mrs. Grimes graduated with honors from the University of Wisconsin Medical School's Physician Assistant program in 1997. She is also a nutritionist with a Bachelor of Science degree in Nutritional Sciences from the University of Wisconsin at Madison.

She is board certified by the National Commission on Certification of Physician Assistants; licensed by the state of Wisconsin and is a member of the medical staff of Watertown Memorial Hospital. Mrs. Grimes' philosophy of patient care is to provide comprehensive medical care that addresses the overall well being of a person. Mrs. Grimes' personal interests include provision of community services through education and evaluation of nutritional or alternative medicine therapies that a patient may seek in addition to traditional treatments.

Her immediate goal is to improve her Spanish to better serve her Hispanic patients. Mrs. Grimes chose to be a practitioner in family medicine because she enjoys the variety of medical challenges that are presented when seeing patients of all age groups. Outside of work, Mrs. Grimes enjoys gardening, reading, traveling and spending time with her family. She lives in Lake Mills with her husband Zach and their daughter Olivia.

Keeping your medical records confidential

What you need to know about Turke Family Medicine, S.C., Confidentiality Policy

Turke Family Medicine, S.C., is committed to providing you with high quality health care and to forming a relationship with you that is built on trust. That means respecting your privacy and confidentiality of your medical information. We protect your privacy and confidentiality rights by creating and putting into practice policies and procedures that allow access to your personal medical information only for legitimate reasons.

Your medical record

As we provide your health care, we are required to maintain a complete copy of your medical history, current condition, treatment plan and all treatment given, including the results of all tests, procedures and therapies. Whether this information is stored in writing, on a computer, or other means, we will keep this information in a safe and secure way that protects your privacy and confidentiality. Of course, the physicians and other health care professionals who are involved in your care need to access this information in order to provide appropriate treatment for you.

Your medical information is private and confidential

You, or anyone to whom you give written permission, or your legal representatives, have the right to read or get a copy of your medical information. Your medical record is the physical property of Turke Family Medicine, S.C.

How do we assure your privacy?

Turke Family Medicine, S.C., has put in place detailed policies regarding access to medical records by our staff and employees and has carefully outlined the circumstances under which your medical information may be released to parties outside Turke Family Medicine, S.C. These policies conform with state and federal law and are designed to safeguard your privacy. Our staff and employees are trained in the appropriate use of medical information and know that it is available to them only to continue to provide care to you or for other limited but legitimate reasons. A violation of confidentiality or the failure of an employee to protect your information from accidental or unauthorized access will not be tolerated. This may include the employee being fired from his or her job.

We ask for your permission

We do not allow others outside Turke Family Medicine, S.C., to access your medical information unless we have the appropriate authorization to do so. We will request your authorization to release information at your first visit or admission. In addition, some laws prevent certain types of patient information from being released without specific patient permission. Examples include, but are not limited to: ¨ Confidential details of: o Psychotherapy (from records of my treatment by a psychiatrist, licensed psychologist or psychiatric clinical nurse specialist) o Other professional services of a licensed psychologist o Social Work Counseling/Therapy o Domestic Violence Victims' Counseling o Sexual Assault Counseling ¨ HIV test results (Patient authorization required for EACH release request.) ¨ Records pertaining to Sexually-Transmitted Diseases ¨ Alcohol and Drug Abuse Records

Please note, however, that the law requires some information to be disclosed in certain circumstances. This includes mandatory reports of abuse of children or elderly or disabled persons. Also, subpoenas or court orders may compel the disclosure of confidential or privileged health information in the context of a lawsuit or administrative proceeding. Medical records are sometimes used for reasons other than patient care. For example, records are periodically reviewed to evaluate the quality of care, or to be sure that Turke Family Medicine, S.C., follows the rules of regulatory agencies for the efficient and effective utilization of care such as Medicare, Department of Public Health, or Department of Mental Health. Your insurance company may request information that we are required to submit in order to provide and bill for your care. Anyone reviewing records must follow the same confidentiality laws and rules required of all health care providers. Patient records are valuable tools used by researchers in finding the best possible treatments for diseases and medical conditions. All researchers must follow the same rules and laws that other health care workers are required to follow to insure the privacy of patient information. Information that may identify you will not be released to anyone outside Turke Family Medicine, S.C., without your written approval. In all research conducted within Turke Family Medicine, S.C., concern for your privacy and well - being is our first priority.

Your rights

You have the right to inspect and copy your protected health information. You have the right to request a restriction of your protected health information. You have the right to request to receive confidential communications from us by alternative means or at a alternative location. You may have the right to have your physician amend your protected health information. You have the right to receive an accounting of certain disclosures we have made, if any, of your protected health information.

Complaints

You may complain to us, or the Secretary of Health and Human Services, if you believe your privacy rights have been violated by us. You may file a complaint with us by notifying our privacy contact of your complaint. If you have questions . . .

*We are required to abide by the terms of this Notice of Privacy Practices. We may change the terms of our notice, at any time. The new notice will be effective for all protected health information that we maintain at that time. Upon your request, we will provide you with a copy of any revisions we have made.
